After installation, you can personalize patient education materials by adding your name, practice name, address, Web site, phone number and the days your practice is open in the tab 1 area. This information will be recalled each time you launch the application. In the tab 2 area, you can also add your patient's name and then click the printer icon next to the documents that you would like to print. You can skip this step if you'd prefer to simply print the documents and write the information by hand by selecting the "Skip customization" checkbox at the top of tab 1.
Click on the download patient education application link below to install the application that contains helpful tools and information about migraine. You can personalize for your patients, and then print out as needed right from your office computer.
